A major step towards interoperability between aged care and general practice is set for early next year, with BESTMED and Best Practice Software announcing an integration agreement that will link about half of Australia’s residential aged care homes with 70% of GP practices.
The collaboration between BESTMED, a cloud-based aged care medication management platform, and Best Practice Software, Australia’s most widely used GP software, promises to transform how doctors manage patients in residential care homes.
The integration will allow clinicians using Best Practice to log in to BESTMED via single sign-on, giving instant access to up-to-date resident charts and medication lists. By eliminating duplicate data entry after aged care visits, the system will streamline workflows, improve prescribing accuracy, and save valuable time for practitioners.

Key features of the integration include:
- Seamless authentication to BESTMED within Best Practice Software
- Automated chart access, enabling secure retrieval and updates of BESTMED patient charts from within Best Practice
- FHIR-based medication data exchange, allowing prescribing data and notes to flow directly from BESTMED into Best Practice
Once rolled out, the integration will be available at scale to more than 1,250 residential aged care homes nationwide.
