What Is coordinated informal care?

At the heart of the app is a powerful idea: coordinated informal care. This model brings together a person’s natural support network – those who want to help but often don’t know how – and gives them the structure, education, and confidence to step in meaningfully.

Rather than relying solely on formal aged care or clinical services, coordinated informal care recognises that trusted relationships, consistent support, and clear communication can make all the difference for someone living with dementia. And just as importantly, it helps protect the wellbeing of the primary carer by sharing the load.

Why it’s needed

Every day, thousands of Australians care for someone with dementia. Many do it alone, without guidance or backup. But evidence shows that when care is shared and coordinated, outcomes improve – for the person with dementia and the carer.

The Caladenia Dementia Care App addresses this critical gap by:
